• I am a first home buyer.
• I am an essential worker (eg. early childhood educator, teacher, nurse, midwife, paramedic, police officer or other essential public service).
• I am a single parent with a dependent child or children.
• I am a survivor of domestic abuse.
• I am experiencing housing instability due to separation, divorce, or other adverse circumstances.
• I am experiencing housing insecurity due to the lack of available affordable accommodation in the area where I live (eg. I have been required to move regularly, I am struggling to find suitable accommodation due to low rental supply and high cost of living, I am experiencing rental stress, or similar).

By submitting this form, I confirm:

• All the information provided in this form is true and correct.
• I will provide evidence to support any of the information provided in this form on request.
• I understand that participation in the EquitiShare program is subject to further eligibility checks and additional lending criteria to determine what amount I can borrow.
• I consent to the provision of my details set out in this form being provided to suppliers who provide services to Sunrise to assist it with delivery of the EquitiShare program in accordance with its Privacy Policy, including specifically to any real estate selling agent authorised to sell Sunrise property, and any affiliated broker and/or EquitiShare lending provider to complete initial credit worthiness checks and initiate further contact to assist with my participation in the EquitiShare program and any loan application, subject to the program lender’s Privacy Policy which I have read and accept.
